Fill out one short, secure form. We collect the basics - your name, date of birth, phone, email, and insurance card. Takes less than 5 minutes.
When we match you to a provider, we send your information directly to them so you're not starting from scratch on their intake paperwork. You've already done that part. Just show up. -
We review your request within 24-48 hours and connect you with up to 3 matched providers depending on availability and matching. No guessing, no cold-calling a list of names.

Your matched providers (up to 3, depending on availability and matching) reach out to get you scheduled - no cold calls, no voicemail roulette.
We check in at Week 1 to make sure you got connected, and again at Week 2 to make sure you’re happy with your match. After that, your care is in your provider’s hands, exactly where it belongs.
